Treatment Agreement & Consent

This document is provided after you book and pay, and before your first session begins. It is not required to view services or booking pricing โ€” it is a client-facing consent form, not a marketing page.

Non-emergency service

Let's Talk Respiratory provides education, coaching, equipment support, and non-emergency respiratory therapy consultation. It is not a substitute for physician care, hospital care, or emergency services.

If you are experiencing respiratory distress, chest pain, confusion, blue lips or fingertips, or any other medical emergency, call 911 immediately.

Scope of service

  • โœ“Equipment education, setup review, and troubleshooting (CPAP, BiPAP, oxygen, nebulizer, ventilator, tracheotomy)
  • โœ“Patient and caregiver training
  • โœ“Referral guidance to your primary care provider or specialist when appropriate

Not included: prescribing, adjusting settings that require a physician order, diagnosis of new medical conditions, or emergency response.

Self-pay & refunds

All sessions are self-pay. Insurance is not billed. Prices are listed on the Services page and shown at checkout. If you need to reschedule, please give at least 24 hours' notice through Calendly.

Confidentiality

Sessions are private. Records are stored securely. Information is not shared without your written permission except when required by law.

Consent to session

By continuing with your booked session, you acknowledge that you have read this agreement, understand the scope and non-emergency nature of the service, and agree to the self-pay terms above.
